Medication Administration Permission Form for OTC and prescription meds to be given at school. Meds must be brought in by an adult. UME Prep does not provide any medications.

NOTE: Over-the-counter medications, even with parental consent, must be unexpired, in their original container, and properly labeled. These requirements help ensure student safety, as administering expired medication does not meet accepted safety standards.
